Accidents come down to poor judgement of the situation. Either before or after the accident. Then the excuses. Good judgement prevents almost all accidents.

There is a signature line that addresses this; something like "Poor judgement causes experience. Experience causes good judgement."

I agree, but only in general terms; for one example, some "accidents" are caused by a mechanical or electrical failure that cannot possibly be anticipated.
