Can you explain how this can happen?

I think it is very unlikely.
To have a stuck open injector causing diesel to leak into the crank case is very unlikely. It would need gravity feed of diesel, pump piston for that cylinder in the "down" position, a stuck open delivery valve, and a stuck open injector (when parked). And when started an engine smoking and running badly.

Good run down Bert.. funny but true.. BUT it does happen..
BAD FUEL gets all the way to the injectors, sits & gets worse..
THEN, when the engine is cranked.. the pintle/center of the nozzle, sticks open..
I see it just about weekly.. I pop test an injector, it sprays 1 time & sticks open..
90% of fuel shops will just replace the tip/nozzle.. where I take them apart & clean them.. 20.00 vs 90.00
