Well I must admit you've had more than your fair share of failures with that hub. I've been through a number of them and have never had repeat failures to that degree. A few problems where I didn't get the nut tight enough, but none resulted in any gear failures.

In the case of the photos just posted the axle half shaft (at least) had to be replaced, meaning the spindle housing was removed. Are you fairly confident that was installed correctly and the trunnion bearings were properly shim adjusted? That's a long shot, but I can imagine a possible issue there.

This axle does not have shimmed trunnion bearings, but I am thinking that the problem is improper assembly of the hub.

It could be that the shaft bearing carrier was/is not fastened in the axle tube by the set screw on top of the axle housing.

Improper tightening of the axle nut will allow for for wear and then sloppiness in the bearing/hub/planetary which will result in gear damage.
At least I know it will on a T6050.
On the farms 6050 when it had issues the first time from getting trash and garbage (tall grass and weeds) wrapped around the hub which wiped out the seal and allowed dirt to get in the housing which wiped out the bearings before the play and wobble was noticed in the front wheel.
After the initial repair the nut was not tightened properly (done in the spring), it was tightened with a hammer and punch then backed off like a normal wheel bearing.
It failed by mid summer a more aware operator noticed some play in the front wheel, lifted the front end with the loader and could wiggle the front wheel slightly, the decision to complete work that day in 2wd was made, on the way to the home farm the wheel was noticed to be extremely loose and travel was stopped until the front axle was suspended for the rest of the trip. The following repair was quite expensive and did involve replacing the pinion gears and the ring gear as well as seals and bearings and couple of bushings. When that repair was completed and the axle nut tightened as I previously noted it has been good for several years now.

No sir I cannot be certain. I was depending on the mechanic to make all the fine adjustments.
