2012 Kubota L4400 with PTP clutch worn out

I have a 2012 Kubota L4400 with only just over 200 hours. The PTO has been used very little. However, the tractor is in the dealership now to replace the PTO clutch. Does anyone have any information to help on this. The dealer said something about a recall on this clutch few years back. He also said the clutch plates were warped.
What would cause this problem?
