Saw a guy on youtube who welded a tap (as in Tap & Die) of a slightly larger diameter than the opening in the cup ('Course' thread would be OK) onto a metal rod of about 12" and fashioned a sort of slide hammer from it. To use he just threaded it a ways into the cup and pulled the cup out. As for when/if to replace, most of what I've learned says only if the cup is cracked or otherwise damaged and cannot be cleaned up do you need replace them. I've found a wooden golf tee, one 4" or longer ('standard' length tees are too short to poke past the bore opening, making them hard to pull out without needle nose, etc.) plugs the hole nicely so you can clean any carbon and blow it out without debris dropping into the cylinder.
