If the injection pump is starved for fuel, the plunger won't be able to deliver the full "shot" of fuel to the injectors at the required, steady, pressure, which results is poor atomization and incomplete burning of the fuel mist. Specifically, the fuel in the center of the big droplets won't burn at all, producing smoke..
