ford 7840sle clutch problem

hi all.new here to this site.i have a 95 ford 7840sle and it is hard to select gears.its like as if clutch needs the slightest adjustment.at every gearchange it grinds harshly into all gears.oil and filters are less than 12months old.i have tried calibrating clutch packs but makes no difference.can somebody please put somelight on this problem!!!!
