Most NAPA parts stores makes hoses, just take the old one with you so they will know how to build it.
The parts house will probably know of a HYD shop that can repack the cylinders for you, they probably can make you a hose too.
Not sure of your location, but I don't think I've ever heard of a hydraulics shop in the smaller towns I've lived in. Tractor dealers or autoparts stores that make hydraulic hoses and sell fittings is the closest I've seen.
