JD 214 Mower Deck Belt Keeps Twisting

I have a JD 214 and after the PTO/mower deck have run for a while (2-20 minutes) the belt twists so that the V is not in the PTO groove (it's "upside down").

Can anyone suggest possible causes/solutions?

Maybe like a tensioner arm is bent or worn so the belt alignment isn't good?
