My size 12 boot in not big enough?

It has happened only twice but today it really scared me, some how my foot slipped between the the two pedals and got stuck, going forward , I couldn't pull my foot off the fwd pedal as it was wedged between the pedal and It was fully depressing the fwd pedal, I just turned the key off, but it made me nervous as I kept moving.
Is this just me or has anyone else had this happen?

What if you made the pedals wider. Kinda like these pedal covers you see kids pimping out their rides. Just a thought. -kid
