Found this in a tool room at work. What is it.

It appears to be a multi-head screwdriver/nut driver combo with some attachments missing.

The hex part turns independent of the sq. part. Inside the neck is threaded with real fine threads.

does the hex part slide in and out of the square part? In other words, could it work as a slide hammer to remove something it threads onto?

It moves about a1/2" is all.

without dimensions, it's hard to tell how big it is. But.... could it be something like this:

used for settling concrete while it's being poured.

That's it exactly. We don't have the hose to use it. It is the most petite vibrator I have seen. The threads that screw to the hose are super fine. I can't imagine it getting dirt and concrete in them
