I believe it is a fire hose restrictor, that you clamp on the hose to slow the flow of water.

I would guess it goes over a round electrical conductor and is tightened down and a cable is connected on, ie power from a overhead line.

You came the closest,but it is not for setting broken bones,altough a limb is involved.Looks to dirty but how about something medical, like setting broken bones in a limb? wild guess!
With the large open part and the small fork/yoke, it has to hold something small to something larger?
