Lockout rules were around in those days but many companies now have much more strict policies. As an example multiple people can padlock off a unit and no one can start it until everyone who padlocked it off takes their padlock off. In the early days it was just a tag.

When I was working, I had 6 lockout locks, for multiple sources. To remove my locks, without me being there, required the presence of the safety rep, supervisor, area supervisor, and a business unit manager. In writing. We had a supervisor get fired for sending a operator into a machine enclosure without being locked out. Nobody got hurt, but the supervisor still lost his job. I'll bet he won't shortcut that again.

I worked for a local company. Day shift had locked out a hydraulic man lift because it was not operating correctly. Our night shift supervisor simply ripped the tag off the lift and told employees to go ahead and use it. Fortunately, no one got hurt - also fortunately, the night shift supervisor lost his job over that stupid stunt.

There was no actual padlock, just a tag? The company should review their policy, assuming this was in recent years.

Yes, all we had for lock out-tag out were paper tags to write on and they were attached around the equipment "start up" button with string. In that way, no one had to go to the "inconvenience" of finding keys to remove the tags, etc. I last worked for the company in 2001. When I quit, I felt very fortunate to be able to walk away with my life and all limbs functional and not injured.

No, In Fargo it was a kidnapped woman that would not "shut-up" The kidnapper "cracked" and the chipper did the rest. The wife of a car salesman.
