OP here . Just got a second notice in the mail . Said 2nd notice due Feb . 2 . I did not respond to the first one . I have highlighted and circled the part about voluntary on the form and written " I choose not to volunteer " . I haven 't decided to mail it or not . Maybe I won 't and just see what happens next .
Anyone get on of these government surveys ?

I got another one that said "past due", still in Mom's name, so I circled her name & wrote "deceased" on it and sent it back!

Only a small percentage of farmers get the questionnaire, just enough to make the responses statistically reliable. There is no reason not to fill it out and return it. I got one every year for 3 years running, but haven't received one since 2012.
