Just reform the sides and make the form level and sloped like you want it. Brush on a binder and mix up the self leveling concrete. Pour the entire pad up to to the side forms and let it set up. Now the only part that will be noticeable will be where the old pad and new pour meets. But it's on the side and just back fill with dirt.

I tried doing a skim patch on a set of stairs at a rental house of mine, where a renter put way too much salt down. It did not work out like I would have hoped. I ended up doing what I outlined about, but had to do it stair tread by stair tread because of the forms. It worked well and I put a very course brush effect down once it set up enough for extra traction.
