Do you think this is a virus/worm/trojan?

I have tried rejecting all cookies. I do use Spybot S&D, System Cleaner, Ad-Aware 6.0 in addition to Norton wirh automatic updates enabled. We are operating behind a router. About ready to clean out and re-format the hard drive as a last resort--Ken Sweet
