Finally figured out it's a Malm Fire Dome 2 made in Santa Rosa, CA.
Installed it with a few modifications after it sat for about 25 years and was only fired up once.
I had to make a reducer section to go from 9" to 8" pipe, cut out some ventilator holes in the chimney pipe, adapt the 8" pipe to 6" at the ceiling and them assemble the 500 lb. stove piece by piece. I also added a piece to the diffuser to give it better heat transfer. Before all of that I pulled out the old stove.
