I've never seen a nose wheel collapse incident in person, but I've seen about half a dozen of various models of aircraft where someone forgot to put the landing gears down.

I used to get called in to work in the middle of the night to tow them off the runway once the safety crew raised them up and got the gear down.

When I was a kid working at the airport back in the early 70's we had a guy come in in an early Bonanza with the piano keys and got the gear instead of the flaps on rollout. Caught it before the mains went, but I bucked a lot of rivets on that one.
Back in the 80's when Salem had slf ops, Horizon was running Metroliners. don't recall any incidents.
