Rotor is right. That bolt pic is a signature failure cause by overtorquing. In my opinion its a slow overtorque, not a high speed stripping caused by a pulsing impact wrench. Bolts are too soft. An impact would have snapped the head off. Are there supposed to be tapered pilots on those lugs instead of a flat head? Also, is the wheel piloted by a hubcentric ring? If so, the bolts just don't really need to be that tight. Your other wheels ought to tell the tale. They are probably in the same condition. Another thing to watch is the thread pitch. This could be an English tread in a Metric hub. It works for a while. Foam is for junkyard operation, not traction.
