Rebuilding a RCK54-15BX, but would be common to same design.
The tension arm uses a clip to keep the tension pulley up and off of the arm itself.
On my wife's mower, string got under the clip, wound up tight in the groove, and lifted the clip out of its groove on the shaft, but stayed on the shaft.
The pulley 'fell', pushed the clip down the shaft, and the pulley followed it on down.
The pulley rode on the tension arm long enough to cut about 1/3 way through, and cooked paint for 2 inches.
If you see your tension pulley has touched the tension arm, I would be suspicious of the same thing.
