If you haven't already I would do what chriswheeler said and loosen each injector line while it's running. This will pin piont a ceartain cylinder if it's an injector, head gasket, valve or piston/ring problem. You will have a better idea of what to look for and where to start. If it does point to a specific cylinder you can put air pressure in that cylinder (TDC on the compression stroke) listen for air leakage, and look in radiator for bubbles
