I made this one quite a few years ago. Works pretty good for me.

Here's mine. Put it on my most used implement, the forks. The close up pic shows the lower ball and the distant pic shows the upper, gooseneck ball on top.

They have draw bar goose neck hitches and adapters for the other hitches

She's not pretty but she'll work. I'll probably weld a 2" and 2 5/16" ball on each side of the reciever later.
