Just had mine pumped earlier today. Pumpers canreach out a a long ways if needed, my neighbors tank is about 100 feet or more from his driveway and they have no problems with it. With mine, it's not very far about 35', they do park the truck up the hill a bit and probably lift it about 15', don't know what the upper limit is, probably 30 ' or less.
Around here they haul into various municipal treatment plants to dump. We used to have a company that had their own treatment lagoon that they would then apply to their own fields. Sprayed it over their property line and the neighbors filed a complaint. State or county (I don't remember which), shut them down almost the next day. I think that they were looking for a excuse to shut them down. There was a level of worry since they hauled out of Tacoma and served business as well as homes, some talk of heavy metal contamination.
Cost here is about $500 plus $150 to the county for the operating permit (3 years). Not cheap at all.
