I think he used to turn hard with a full load in the FEL and possibly hit ruts, kerb's and the like at full lock to break out the bolt holes like that 
Yes, likely. I've had the steering wheel jerked out of my hands going backward turning, and catching a rut, causing the steering to hit its limit hard. I suspect that's what he did. And apparently he did that often.
