It goes to show how much you need to watch out for your own safety and not rely on others. Certainly, Caterpillar should have had some safeguards in place here.

My oldest brother (RIP) worked at Bethlehem Steel many years ago. He once told me that when this happens, though rare, they tap an ingot in the approximate weight of the person for the burial. I guess his spirit is still in there.

A guy at a company near here (one that I previously worked at) fell into a vat of molten salt brine used for heat treating large racks of parts. He was adding salt to the brine tank (1700deg approx), slipped, and fell in. When I was there, I don't recall ever seeing guard rails, safety harnesses etc around the heat treat areas. I would imagine that their methods have changed since. Same thing with stacking large coils of strip steel. Tall stack that was leaning precariously, fell over, person nearby killed, same company. This would be in 2014 or so, so not that long ago.

In the late 60's when I was in high school I worked at Inland Steel in Gary, IN. Worked there for a couple of summers. Inevitably, at least once a year, somebody fell into the vat of molten steel. Sad, but it happens.
You can put up all the safety guards you want, but accidents still happen. And not because of stupidity.
I agree with basic safeguards, but not the safeguards to protect idiots from themselves. i.e. warning labels on fish hook pkg that says "do not swallow".

At a stud mill years ago a crane operator sat in his machine watching helplessly while his coworker crawled into a whole tree chipper to work on it... not realizing that the drum was still turning. That's got to be a tough thing to get over.
