Here in BC, workers comp (WCB) has been simple in my experience.  I got hurt, the company paid the wage for the day.  Dr billed WCB directly and got paid.  WCB paid 75% of wages tax free (no pension benefits paid) starting the next day either directly to me or to my employer and I got a regular pay cheque.  
People do get denied claims and there are review processes set up. There is also a fraud dept that reviews claims and they do have cameras to get those pictures.
