I clean the internal threads of old teflon tape using the bent end of a machinist's scribe. Just start at the inner end of the threads and run the scribe around the threads counter clockwise. It pulls the old tape out pretty well. You'll have to carefully pull out any small pieces.

The only sealer I've found that works as good as teflon tape is LockTite 242 thread locker, but everything has to be oil free and it has to dry.

I used an old electric shaver cleaning brush, given it was small / stiff. Then kept tipping the cylinder so the slight flow kept flushing the tape debris out as I picked it off the threads. Pretty much as explained in the above responses and in the end way more simple than expecting it to be. Thanks for all the inputs.
