front wheel

I have a 1989 deutz allis 5220 tractor with 2150 hours on the counter. While cleaning the snow the left side front wheel fell off. There doesn't seem to be anything broken but can't find the problem. The shaft, the threads seems fine , the threads look OK. What could have caused the wheel to fall off !

I'll assume it's 2 wheel drive..?? Sounds like the cotter pin came out, and let the castle nut back off the threads. I looked up the assembly at AGCOPartsBooks. Most everything I've ever taken apart has some sort of thrust/keeper washer behind the nut. Apparently this model doesn't. Driving forward could I suppose back the nut off eventually.

What does the outside bearing look like..?? I did have an outboard bearing cage come apart on me, on my tri-axle trailer, and lost a wheel off the front axle a few years back.

Has someone had the left front wheel off, ever..?? Maybe they forgot to put the cotter pin back in..??
