I used to carry a 1911 Colt cocked and locked for years, I got old and it got heavy or both.
I'm not sure how a pistol goes off with a hug, I'd like to see more info on that.
Not carrying with a round in the chamber? Do people still carry a revolver with and empty chamber under the hammer? Not normally. Most semi autos are designed to be carried with a round in the chamber but its up to each person on how to carry a weapon.
I did a look because the story is old. This must be the source, or a source of the original story I heard. It was in the news all over at the time.

Unless everybody thinks they are a gunfighter, why carry with a round chambered?
I dropped the chambered round when a Sherif with a holstered M&P in a Galco shoulder rig had a round discharge when he was hugged at a wedding and it killed the woman who hugged him from behind. I'll take the risk of the extra half-second to rack the action.
FWIW, I like the Bianchi 56 Serpent Holster which keeps everything nice and tidy.
