I had to apply for the programs every year. Now's the time to do it. I'm surprised your cousin's doctor didn't mention it.
On the flip, why don't you give one of your kidneys to your cousin. My mom did that for my brother. She was 60 at the time. Mom will be celebrating her 95th birthday in January. My brother passed away at age 50 from other complications. He lived 30 years more than he was expected to at birth.
