A story of similar: I was scheduled to visit an overpass widening project in L.A., just a routine project-docs review. The evening before, the news showed, with dramatic photos but not much explanation, that a 'crane operator' died at the project. I called the job trailer and left a message cancelling my visit. These examinations of project-supervision records are random for quality control and I could go bother someone else that day.
Later I re-scheduled that visit and heard the real story.
An unrelated crane owner was driving down the freeway with plenty of clearance. But his unsecured mast bounced up out of its cradle and snagged the last temporary girder, 'falsework', under the concrete forms for the upper road's widening. He died when the girder crushed his truck cab. The end.
