Just tried to order two more hats but for the life of me can not get it to work. The last time I sent the form directly to you and now it says to fax or mail. What am I doing wrong?

Paul,

When you are on the checkout page, where it says "How would you like to pay for your order?" make sure you select credit card and not printed order form...
