Compared to many activities, railway regulations are ridiculously outdated..... historic legacy inertia....... I'm not a fan of over-regulation, but where I want to see it applied is to Life-Safety.

With telemetry available for decades, pipelines can be continuously monitored - in fact, in today's world, I'd design a bulletproof monitoring system - Invite Greenpeace and Sierra et al to select a 3'rd party auditing company to review it (I'd pay for the audit), then commit to having alarm information posted on a public website. But.... that takes pragmatic logic, which seems to be in short supply....
