Also they are called a leveling box , if you want to search for them, many after market ones out there.

I was able to get a couple of quick measurements, mine has three sets of holes on the lower mount that hooks to the lift arm itself, so from the center hole there to the center of the top mounting hole is 16 inches, and the upper hole is 3/4 inch diameter. This was with the arm at approximately centered in the adjustable travel, If i can get any other info for just holler. Hope this helps, Also this is on a L3410.
