ruffdog - as I remember, there was no form of safety harness involved. Subsequently they(the company) did come up with a harness arrangement. It involved strapping off to the overhead piping, as I remember and not to the hydraulic lift. After that situation, I seriously doubt that the company could have got anybody to harness themselves to any hydraulic lift. When that lift came down it made a real mess. Hydraulic fluid everywhere and parts of the scissor arm assembly broken and scattered everywhere - it wasn't pretty.

In most cases OSHA requires you to have a safety harness attached to the lift as falling out of the lift is a much bigger threat than hydraulic failure. The simple fact is that most common cause of injury and death from falling involves ladders. I understand other people have different experiences and concerns but a hydraulic hose failure is no more sudden or dangerous than a ladder footing giving way.

And that's why I built my work platform. To mitigate the hydraulic failure issue, I put a 4x4 timber under one of the lift arms; the same one I use when I need to lift the FEL to service my tractor.

A lot of those manlifts are considered mobile scaffolds. If they have the proper height railing all the way around, OSHA does not require a harness, though your company can require one.

Hydraulic cylinders for personnel lifting also have orifices screwed directly in the cylinder to control the fall speed if a hose does fail. But that is not going to stop a mechanical failure, which is what sounds like happened in the previous story of the guy hanging on the conduit. If the lift was "in pieces" below him.

Try not to hit the end of the lift stroke when someone is on the loader platform. The spike in pressure would be the most likely cause of a hose failure.

Pallet forks with a matching grapple, it does everything two separate tools will do. Add a apple box and you have a GREAT platform to work from,

The grapple locks it into place so it's safe, it get's used for a lot of jobs around here...

My variation on a familiar theme. This one is held together with c clamps because it offers me another level. I can run it with just the bottom basket or the top basket fastened to the bottom. It works just great for us. I hold it fast to my forks with either straps or chain and binder. It is so handy for everything including a cleaning of the owl box at the end of fledging.
