I have a Ruger Mark ll target with a bull barrel. An extremely accurate gun when shot off sandbags. A little more accurate than the my K-22 when shooting off of sandbags. When shooting off-hand I can shoot the K-22 much better than the Ruger target model. The K-22 is balanced perfectly and fits my hand so much better than the Ruger.

A significant issue I have with a .22 rim fire is reliability. Okay.. I shoot cheap brick packs but I have had 20 FTF to one FTF center fire 9mm. Actually..... I have never had a FTF in 9mm. misfeeds and FTE but never a failure to fire. And I've shot thousands of the cheapest 9 mm I can find.

Perhaps a .380 auto will please your wife.

You are correct, that is the one drawback of .22 rimfire, the misfire.. but you can mitigate that somewhat by using the higher quality rounds. There is a difference.. The cheap bulk packs have the most failure to fire rounds in them. Factory center fire rounds, rarely don't go bang. I have seen a few reloaded center fire rounds not go bang the first time, but it is usually because the primer was not seated correctly and the priming pellet was not crushed enough to sensitize it. Or.. I have seen a primer seated backwards a time or two...ooops.

The misfires that I have had with .22 rimfire ammunition are so very rare that I don't even consider that a factor. I buy CCI Green Tag first if I can find it. And Remington Target when I can't. These don't misfire.

I'm also an advocate of those small caliber semi-autos with tip up barrels. For a person with limited use or strength in their hands, it's a viable option.
Personally, I'd go with a center-fire (that is, .32ACP or .380) rather then a rim-fire.
