I spent 26 years as an engineer for US auto company including a stint in the emissions and fuel economy area. We had separate departments to develop emissions systems and to validate their performance. Compliance progress was tracked at the VP level. I agree. VW had ggressive top down leadership by highly accomplished, detail oriented engineers for 20 years. But a handful of rogue engineers did this on their own - across several brands and on several continents - and nobody knew. Nobody asked, Gee, how did you get all that performance while meeting the emissions standards. Some one will write a book years from now and sort it all out.
