Glad it was something simple, I'm kinda like Hay dude I believe I would examine every aspect of how I was getting/storing/filtering my fuel to help eliminate this from happening again. Let us know if we can help, we are full of advice/ideas :D .
I feel that I am doing a lot to avoid fuel problems.

I add Power Service Diesel Fuel and Tank Cleaner to every 100 gallons I put in my nurse tank. Clear-Diesel Fuel & Tank Cleaner

I have a 'water block' filter on my nurse tank.
The suction end is about 1-2" off the bottom of the nurse tank
I check the water separator for signs of water almost every time I start the tractor. (there were no signs of water this time)
I buy diesel to fill the nurse tank at different fuel stations. (I look for lower prices).

I really don't know what more I can do. I guess I could add another filter as @5030 suggests.

I welcome any advice.

All the additives in the world can't do everything, also any additive that absorbs the water to carry it through the system is going to be alcohol based. So you are running water and alcohol through your injection pump.
I prefer to leave one of these in the bulk tanks.

I've never heard or seen this. How often do you replace it?

I see no mention of a biocide. i would skip the diesel tank cleaner and use biocide instead especially if you have had problems with water in the fuel in the past. keep extra filters on hand after treatment because you will go through them. I use BIOBOR JF it is the only treatment approved by the FAA and not too expensive. I put 1.75 ML in a 5 gallon can. One teaspoon is 5ML so my bottle will probably last 20 years.

If you have a pump on your tank you can put the nozzle back into the tank and run it to "polish" the fuel and stir in the biocide. Can you drain the bottom of your tank.

I have an ultramar commercial pipeline fuel station right off the highway where I live all the trucks go there so I figure the fuel is fresh. chasing cheap fuel may not be the best idea in this case

I think the company gets a break on the cost because they get it by the case lots. I have to buy my own from Amazon.
Edit: I have a 50 gallon bulk tank that I sample monthly.
On the ship the fuel is also centrifuged. Here is dead bugs the centrifuge caught.
