I have to have the engine running to align the 3 points. Once the implement is hooked, I shut off the engine, make the necessary adjustments and hook up the PTO. Done that for several decades without any problems.
I've always turned the PTO shaft on the implement to align it.
I never hook up my PTO implements with the tractor running.
I don't mind the extra steps required.
The other day my SIL was hooking up the rear PTO on his new tiller. He left the engine run. I told him I "normally" shut down the engine when I hook up the PTO. Safety first.
How about it....do you shut down the engine when attaching your rear PTO implements?
